A Galvan account and wallet are REQUIRED in order to be eligible for rewards.
All values are calculated in U.S. Dollars (USD). Conversions from other currencies will be recorded based on their USD equivalent.
How many memberships in the Health Supplement Rewards program are available?
There are 4 tiers of memberships to choose from and each has limited quantity.
Common: 500
Rare: 250
Epic: 100
Legendary: 50
Does my NFT have to be activated?
Yes, your NFT does require activation and the 90 day time frame on the NFT will start on date of activation. Only one NFT per collection can earn rewards at a time, be sure to only activate one NFT per 90 days. Once the NFT is activated, the 90-day duration can NOT be paused or restarted.
How much do I need to spend on supplements to qualify for IZE rewards?
Depending on the tier of Health Supplement Rewards you purchased you will need to submit proof using the g+ app.
Common: Requires $50 of qualified receipts submitted per 30 days.
Rare: Requires $75 of qualified receipts submitted per 30 days.
Epic: Requires $100 of qualified receipts submitted per 30 days.
Legendary: Requires $150 of qualified receipts submitted per 30 days.

How do I submit my proof of purchase?
All proof of action is now submitted directly through the g+ by Galvan app.
1. Open the g+ app and tap Rewards
2. Tap NFTs/Proof of Action
3. On the next screen you'll see your Active NFTs, NFTs available for purchase and Expired NFTs
*Some NFTs may not appear in the g+ app. You can always view your full wallet balance and holdings at app.Galvan.health/wallet
4. Tap the NFT in the Active Section you want to submit proof for
5. You'll be taken to your Progress Dashboard, showing: required submissions, submissions completed, potential IZE rewards, and claimed earnings.
6. Tap Upload Proof of Action
*The first time you submit proof you will be prompted to allow g+ access to your camera
7. Take a photo, then tap Use Photo
You will see a message that says "Uploading your proof" followed by
"Your proof of action has been successfully reported"
Important Notes
- Submissions do not appear instantly. All photos require review before approval.
- Reviews and approvals typically occur within 24 hours during normal Galvan business hours (9am–5pm MT, Monday–Friday).
- Once approved, your submission count will update automatically.
- You can tap Action History to view approved submissions.
- Non-approved submissions will not appear in Action History.
- If a photo is not approved, you’ll receive a notification asking you to submit a new one.
- A g+ subscription is not required to submit proof of action.
- Ensure the date of purchase on receipts is legible in the submission photo.
Does my receipt have to be from a certain time frame when I submit the proof?
Yes. Each receipt must be dated within 30 days of the date you are submitting it for. Outdated or reused receipts will not be accepted, and receipts from previous months (for example, submitting an August receipt for January) will not meet the purchase requirement.
Can I submit all of my proof at the same time?
Yes, each submission of proof must be reported through the g+ app separately, multiple submissions can be sent in if the required minimum spend was split between multiple purchases.
For example, if you activated the Health Supplement Rewards - Rare on August 10, 2025 you have until September 9, 2025 to submit your receipts that prove you spent at least $75 on health supplements. You can submit one receipt for $70 and a second receipt for $40, therefore meeting the minimum requirements.
Which products qualify for the Health Product Rebate Program?
To qualify, purchases must be health supplements. This includes a wide range of over-the-counter wellness products such as:
Vitamins
Probiotics
Magnesium
Weight-loss supplements
General nutritional or wellness supplements
Other similar supplement products that support health and well-being
Which products do NOT qualify for the Health Product Rebate Program?
The following items do not meet the requirements and will not be counted toward your monthly supplement spend:
Prescription Medications
Grocery items, food, drinks, or snacks
Personal care products (shampoo, toothpaste, cosmetics, etc.)
Household goods or general merchandise
Non-supplement wellness products such as essential oils, skincare items, or beauty serums
Any receipt that does not clearly list supplement products
Outdated or reused receipts from previous months
When will the IZE I have earned be added to my Galvan wallet?
Earned rewards will be distributed to your Galvan wallet on the first Friday of the month. If your last qualifying submission is ON the first Friday of the month, the earned rewards will be sent the next month.
How is the price of IZE calculated for my Health Supplement Rewards?
The value of IZE rewards is based on the value of IZE on the Thursday before the first Friday of the month, as reported on Elevate. This approach helps smooth out short-term price fluctuations and ensures a fair and consistent calculation when distributing rewards for gym activity.
Why have I not received any IZE rewards after I purchased my Health Supplement Rewards program?
Users must complete the Proof of Purchase requirements through the g+ app. If the Health Supplement Rewards Program was purchased and the requirements for rewards were not met the user will not be eligible for rewards. Rewards will be distributed on the first Friday of the month. If your last qualifying submission is ON the first Friday of the month, the earned rewards will be sent the next month.
When do I start earning rewards after I purchase the Health Supplement Rewards?
Membership begins on date of activation. Each 30 days the required minimum purchase (with proof submitted) must be met to receive rewards.
What if I don't have a Galvan wallet set up?
If you do not have a Galvan wallet set up, we are not able to transfer your rewards to your wallet and you will forfeit your earnings. You can follow the instructions here for how to set up your wallet.
What is the total value of rewards I can earn each 30 days?
Users must fulfill the minimum requirements, as explained above, to be eligible for rewards. Once completed, rewards are sent to your Galvan wallet. Based on the tier purchased, the IZE reward amount will vary.
